NH Mom Hub connects New Hampshire families facing perinatal mental health challenges with care that fits them. As the program moved under Dartmouth Health on a state contract, there was no shared platform behind it: no public directory for families, no way for providers to keep their own listings current, and navigators matching families to care from spreadsheets and memory. Dartmouth Health needed all of it live the day it took over the program, without placing family health information in systems not built to hold it.

NH Mom Hub is Dartmouth Health's statewide perinatal mental health program in New Hampshire. It helps families facing depression, anxiety, OCD, PTSD and postpartum psychosis find care that fits them. When the program moved under Dartmouth Health on a state contract, there was no platform behind it, and it had to be live the day Dartmouth Health took over. Built on Softr and Airtable in two months, the platform serves three audiences in one place. Families use a public directory of 679 resources that they search by what they need rather than by provider type, alongside education pages for five perinatal conditions. Providers register, keep their listings current and report availability through a self-service portal, with every edit routed through an admin review queue before it goes live. The standout piece is the admin console for Dartmouth Health's navigators. It brings provider vetting, a closed-loop view that tracks every referral end to end, and a biweekly availability check that keeps listings accurate without navigators chasing updates. The platform was also designed so no family health information ever enters it: resource data and family records live in separate systems. Custom work went only where it mattered. Custom Vibe Coding components power the directory, the provider portal and the admin console, and an AI Navigator built on n8n and Claude turns a navigator's case notes into a personalized, editable resource guide that can be exported as a PDF. It started as a navigator's own prototype, and Base Flows took it to production. The platform was built for a clinical and administrative team to own. Data lives in Airtable they edit directly, reporting runs in Looker Studio, and it meets WCAG 2.1 AA. Providers were rolled out in waves, the Dartmouth Health team was trained with a seven-video tutorial series and an operating SOP, and Base Flows stayed on for ongoing support.
